Covenant Health Care Center-Batavia, a 99-bed skilled care facility, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the second quarter of 2017.
The report, released on Monday, lists the nursing homes in the state with AA or A violations, which might have caused the death of a resident or could cause death or serious injury to a resident, respectively.
Covenant Health Care Center-Batavia was cited with an “A” violation and fined $25,000 for failing to safely transfer a resident who suffered a fracture, was admitted to hospice, and later died. The facility has requested a hearing.
