Rosewood Care Center St. Charles, a 109-bed skilled care facility, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2017.
The report lists the nursing homes in the state with AA or A violations, which might have caused the death of a resident or could cause death or serious injury to a resident, respectively.
Rosewood Care Center was cited with an A violation in November and fined $25,000 and also fined $2,200 for a B violation.
A B violation indicates a nursing home incident or situation that is unlikely to cause more than minimal physical or mental harm to a resident.
